123Formbuilder Embeds

You can embed a from from 123formbuilder into any “Basic” type page using a tag. The form will appear under the page content (if there is any).

Tag the page where you want to form to appear “123form-xxxx” where xxxx is the id of the form you want to embed.

An example is currently setup on https://ifem.nationbuilder.com/behavioral_emergencies_sig where the page is tagged “123form-6104902”
