module_feedback

This module can be activated by giving the tag module_feedback to a feedback subpage.

The form will collect First Name, Last Name, Email, Mobile phone, Employer, Phone, and a message.

There will also be tickboxes for adding to the SEANZ mailing list, text messages, and hiding post from public.
