Organisation Membership Options (basic)
This is a basic page that will give the user the option to "Pay by Credit or Debit Card" or to "Request an Invoice".

#"Pay by Credit or Debit Card" option will direct user to relevant payment page depending on the value of "organisation_revenue" custom field:
- Annual Income $0 to $ 50,000 - organisation_membership_0_50k
- Annual Income $50 to $100,000 - organisation_membership_50k_100k
- Annual Income $100,000 to $500,000 - organisation_membership_100k_500k
- Annual Income $500,000 to $1 million - organisation_membership_500k_1mil
- Annual Income $1 million to $5 million - organisation_membership_1mil_5mil
- Annual Income $5 million to $20 million - organisation_membership_5mil_20mil
- Annual Income Over $20 million - organisation_membership_over_20mil
