Payment page (donate, 12 options)

Depending on the selections they made earlier in their process about themselves / their organisation. They land on the "payment" donation page where they will donate a set amount.

On submit of individual payment, send user to the homepage.

On submit of organisation payment, send user to "organisation_membership_additional_contacts"
